The U.S. Department of Housing and Urban Development (HUD) seeks to change several Rental Assistance Demonstration (RAD) forms. HUD issued a 60-day notice of proposed information collection that directly impacts housing providers involved in the RAD program. This June 2, 2025, outlines revisions to existing contracts and processing forms.
Why Is HUD Revising the Rental Assistance Demonstration Forms?
The Paperwork Reduction Act requires HUD to regularly review and receive public comment on forms that collect data from housing providers. The current revision targets OMB Control No. 2502-0612, which covers RAD supporting contracts and processing requirements.
HUD’s goal is to ensure all documents reflect up-to-date legal and regulatory standards, especially in light of Executive Order 14168. This directive mandates the removal of outdated references to Diversity, Equity, Inclusion, and Accessibility (DEIA), replacing them with language that aligns with 24 CFR 5.105(a), HUD’s rule on nondiscrimination and equal access.
What Are the Proposed Changes?
According to the notice, the revised information collection includes:
- Edits to Three Key Forms to remove or alter DEIA language:
- Form 52620B: RAD PBRA HAP Contract (Part II)
- Form 52617: Second Component Moderate Rehab PBRA Contract
- Form 5897: RAD PRAC PBRA HAP Contract (Part II)
- Elimination of One Obsolete Form:
- The RAD/Section 18 Blend Rider (Form 5900) is being removed because updated policy allows RAD and Section 18 units to be included under a single Housing Assistance Payments (HAP) contract.
These changes are procedural in nature but essential for ensuring that contract language is consistent with HUD’s current regulatory framework.

RAD conversions must submit comprehensive documentation to HUD. These Rental Assistance Demonstration forms demonstrate that all program requirements have been met and establish legally binding agreements between the agency, HUD, and residents.
The revised forms could:
- Help ensure continued compliance with federal mandates

- Reduce duplication in reporting through simplified forms
